Online booking system for medical practices: options and costs

An online booking system lets your patients book on their own, at any hour, in the time slots you open up. You have four paths: stick with WhatsApp (the main business channel in Argentina) and the phone, use a third-party booking platform, add booking to your own website, or commission a custom system. For a small or mid-sized practice, the best balance of cost and control is usually booking on your own website; a custom system is worth it when there are several professionals, locations or processes of your own.

How do you know you need online booking?

You do not need a big practice for scheduling to become a problem. These are the most common signs:

  • You answer messages to give appointments at night, on weekends or between patients.
  • The same conversation repeats all day: "do you have anything Thursday?", "what about the afternoon?", "and Friday?".
  • Your schedule lives in a notebook or in your head, and when someone cancels you have no easy way to offer that slot.
  • Patients do not show up and do not let you know, and you find out once the slot is already lost.
  • You want to add another professional or open new hours and you worry the schedule will become a mess.

What options are there, and what does each cost?

All four let you give appointments. What changes is how much work they take off your hands, who owns the schedule and how well it adapts to the way you work.

How it worksThe patient messages or calls, and you (or your receptionist) look for a slot. The patient books on the platform's website or app.The patient books on your own website, in the slots you set up.A system of your own with your practice's rules: several professionals, locations, payments, medical records.
CostNo extra cost, but it costs you time every day.Whatever the platform charges: check whether it is a monthly subscription, per professional or per appointment. It is the feature of the Business plan; the cost depends on how many professionals and services you have (see below).Depends on professionals, locations and the rules it has to handle.
ProsNothing changes; patients already know how to do it.Up and running fast, with no development.Your brand, your domain, and patients book where they found you.It adapts to the way you work, not the other way around.
ConsDepends on someone answering; appointments get lost in the chat.Patients book on a site that is not yours and you depend on its rules and prices. There is an upfront cost and you have to keep it online. It is the option with the highest upfront investment.
Who it suitsA few patients per week.Anyone who wants booking right now, without their own website.Practices with one or a few professionals.Centers with several professionals, locations or processes of their own.

The final cost depends on scope: how many professionals, services and processes the system has to cover.

Third-party platform or booking on your own website?

This is the most common question, and both options are valid. A booking platform gets you running quickly without thinking about anything technical. In exchange, your schedule lives somewhere that is not yours: patients book with the platform's design and rules, and if conditions or prices change tomorrow, you have to adapt.

With booking on your website it is the opposite: there is an upfront cost, but the patient who finds you on Google lands on your site, sees who you are and what you do, and books right there, without being sent anywhere else. That builds trust and, along the way, gives people a reason to come back to your website. If you want to be found when people search for your specialty in your area, I explained it in how to rank first on Google.

My practical recommendation: if you do not have a website and need to get organized this week, a platform will get you out of a jam. If you already have a website or are going to build one, adding booking to it is usually the tidiest option in the medium term.

What should a good booking system have?

Whichever option you choose, make sure it has this:

  • Hours and exceptions. You can enter your working hours and block out vacations, public holidays or a day you are not seeing patients.
  • Few steps to book. Choose the service or professional, the day and the time. The fewer the steps, the fewer patients give up halfway.
  • Truly available slots. The system only shows free slots, so two patients never end up in the same appointment.
  • Appointment statuses. See at a glance which appointments are pending and which have already been seen.
  • Reminders. A notice before the appointment helps patients not forget.
  • Looks good on phones. Most patients will book from their phone.
  • Protected access. The schedule holds your patients' data: only you and the people you authorize should get in, with a username and password.

How does booking work in a system I have already built?

Here is a real case. It is from the veterinary field, but the booking logic is the same as in a medical practice.

At Mecohue, a pet shop with a veterinary clinic in Wilde, near Buenos Aires, customers ask for an appointment over WhatsApp, and on the clinic's side the “Today” screen shows every visit (arriving, waiting, being seen, ready to pay and done, including walk-ins and emergencies), linked to each pet's record, with one-tap WhatsApp reminders.

If you also want customers to book on their own from the website, take payment at booking, have each professional set their own hours and days off, or send automatic reminders before the consultation, that is already custom-system territory.

What does booking look like on a healthcare website?

My website catalog has sample designs for healthcare that include a booking section: Sonría Dental (dentistry), MediCenter (a medical center with specialties) and KineVida (physical therapy and rehabilitation). They are not real clients: they are demos so you can see how a practice website with integrated booking could look. If you want to know what a website for your practice includes and how I work, it is all on my services page.

How much does it cost in total to have booking on your website?

In my plans, online booking is the professionals' feature of the Business plan (up to 8 pages plus one industry-specific feature). It has no published price, because it depends on how many professionals and services you have. In the guide to how much a website costs I explain what defines the cost of a site, and a custom web system (with login, admin panels and your own rules) is a bigger project.

On top of that, keeping the website online means the domain and the hosting, renewed every year, with the security certificate (SSL, the padlock) normally included. If you do not want to deal with any of that, there is an optional monthly maintenance plan; I go into detail in how much it costs to maintain a website. Tell me what you need and I will send you a fixed quote within 48 hours.

Frequently asked questions

How much does an online booking system for a practice cost?

It depends on the path. Staying on WhatsApp has no extra cost but eats up your time; a third-party platform charges according to its plans; booking on your website is the feature of my Business plan; and a custom system depends on how many professionals, locations and rules it has to handle. Tell me how you work and I will send you a fixed quote within 48 hours.

Should I use a booking platform or have booking on my website?

A platform is faster to get running. Booking on your website gives you your brand, your rules, and patients book in the same place where they found you. If you already have a website, or will have one, adding booking to it is usually the tidiest option.

Do patients have to download an app to book?

No. A web booking system works from the phone's browser: the patient opens it, picks a day and time, and that is it.

Can I block out vacations or days I do not work?

Yes, and it is something you should insist on: each professional should be able to set their working hours and their exceptions (days they are not available).

When is a custom system worth it?

When there are several professionals or locations, when you want to take payment at booking, or when the appointment has to connect with medical records or other processes specific to your practice.

Your project

If scheduling is eating up your time, tell me how many professionals you are, what services you offer and how you give appointments today. With that, I will tell you which option makes sense for you, even if the answer is to stick with what you have for a while longer.
